I had a glucose Tolerance test and blood work done that tested my Thyroid, TSh, testorne, AMH, TSH, Proclatin, Lipid pane,l FSH, progetsore, then I had a HyCoSy found polyps then had surgery, found Focal Complex Hyperplasia with Atypia the took megace 40 mg for 4 months, then a repeat of HSC, D&C. Found no more pre-cancer and then continue with 5mg if letrozole did not have large follicles, then continie with 7.5mgand trigger shot had 3 follicles but negative pregnancy, continue again with 7.5 trigger sho and negative pre. Continue again with 7.5 trigger shot and negative. Now consider IUI..

Appointments and conversations [with Dr. Pagidas] were rushed as I continued with treatment. I often saw her resident instead of her...Many times I left appointments unsure what would happen next, or what I was supposed to do, if anything...I had an HSG, which showed that my tubes were open, so we proceeded with IUI, despite the fact that the pelvic ultrasound done around the same time showed that I had a 4 cm uterine fibroid. (I didn't know at the time that this would have prevented me from conceiving.) She also prescribed 4 days of Clomid and one Ovidril shot before the IUI.
It was very difficult to schedule procedures [at University of Louisville Women's Healthcare], because it is a Trauma I teaching hospital, so any surgeries had to be scheduled far in advance and you could be "bumped" at any time due to a surgical emergency...It seemed like the only way things got done is if I called and hounded them constantly...The nursing staff was good at returning phone calls and answering questions. I started seeing Dr. Pagidas after my insurance company switched me from another doctor. The first doctor I saw recommended a consult with a GYN surgeon to see if endometriosis was impacting my ability to conceive. Desipte the fact that Dr. Pagidas and the recommended doctor were in the same practice, she never followed through with asking the recommended doctor what he thought. Many times I left appointments unsure what would happen next, or what I was supposed to do, if anything.
What's one piece of advice would you give a prospective patient of Kelly Pagidas at University of Louisville Women's Healthcare?
She is no longer at the U of L clinic, but I would tell someone to get a second opinion, and to stay on top of their care. It seemed like the only way things got done is if I called and hounded them constantly.

The fertility clinic at the University of Louisville is now closed (as of August 30, 2019). It was very difficult to schedule procedures, because it is a Trauma I teaching hospital, so any surgeries had to be scheduled far in advance and you could be "bumped" at any time due to a surgical emergency.

We were provided with a finance sheet for IVF, but never proceeded that far.
Describe Kelly Pagidas's approach to eSET (elective single embryo transfer) vs. multiple embryo transfer at University of Louisville Women's Healthcare.
We did not do IVF, but she told us that she prefers to do eSET, unless there are very few/low quality embryos.

The costs [at University of Louisville Women's Healthcare] were more affordable than other clinics in the Louisville, KY area...Trying to reach others within the clinic is difficult, and has been as Brianna has been on maternity leave. I wish the clinic would have trained someone to take over her place but I cannot get calls back. So long as Brianna there things are great, but not having her really makes it difficult.

She is extremely kind and thoughtful. She has an incredible bedside manner that assures you she is there to help make your dreams a reality.
Describe the protocols Kelly Pagidas used in your cycles at University of Louisville Women's Healthcare and their degree of success.
Doctor Archer got me ready for my IVF, medication protocol. Doctor Pagidas got me ready for my frozen embryo transfer. I had to have the initial ultrasound, trial sonohystergram, begin estrogen, another ultrasound to be sure my lining was good, and then began PIO. Had an ultrasound the morning of transfer and the transfer was in turn completed.

Brianna is simply amazing and makes Doctor Pagidas look good. She is a undergone infertility and her approach indicates her compassion towards her patients. She answers any and all questions and is always timely in her response. She feels like a friend.

Overall Kentucky Fertility Treatment is a great facility and has excellent customer service. The only weakness is the task of getting a phone call answered but they always use their system to give a quick return call... Dr Hunter and his team make you feel like family.... The team at Kentucky Fertility is above and beyond what any customer based business should be. My wife and I feel like the priority every time we walk in the door at Kentucky Fertility Institute.

We used a standard IVF protocol. Dr Hunter, Nurse Powers and Dr Christiansen worked to get a schedule that work for use and a variety of pharmacys to get use the best deal.

Dr. Bohler is an incredibly nice and sensitive man. He listened to me and my husbands wants and needs. He may not look you directly in the eye which can throw one off, but his responses let you know he was taking it all in.... My husband and I did 5 rounds of clomid with times intercourse. They would do a baseline ultrasound between cycle day 3-5. If no cysts, they would prescribe 5 days of 100mg of clomid. I would take that cycle days 3-7. I would start my ovulation predictor kits at CD 9. I would typically get a positive ovulation on CD 14-15. We were instructed to have intercourse the day of the positive ovulation test and then the two days following.
I had questions and emailed the nurse [at Univ of Louisville Women's Healthcare] and got a prompt response. Also, I called with questions and they answered them in a timely manner.... On our first visit we were given the prices for IUI and the cost of our baseline ultrasounds, everything we talked about that was financially possible for us. Well, until we saw the cost of IUI.

I would probably tell them to take it all in and just feel him out, see if he is for you and your needs. Everybody is different and have different experiences. But as a RE, I personally think he is a great one.

I feel like Dr. Bohler really listened to me. When it came to finances, he tried to help us as much as possible. It is a teaching hospital and the courtesy of all the Fellows and residents was nice as well.

The beginning process is very tedious and stressful. Everything has to be in order to proceed. It takes time and I was just thinking “I’m ready to get pregnant ASAP” not knowing the beginning process was so in depth and a little long. Hang in there until you complete that and it was smoothe sailing from there.

He's [Dr. Steven Nakajima] the head of the clinic (or was when I was a patient) so he's busy and it took a while for my first appointment. It was worth the wait. It's also a teaching facility so you speak with the fellow then Dr N and that can seem repetitive and annoying initially. You will see/speak with the other RE's in the clinic based on their schedules but Dr N always knew what was happening with me.
The clinic [University of Louisville Women's Healthcare] moved facilities when I was a patient. In fact, I was the first IVF at the new facility. It's nice and provided much more space. As this is a university clinic, there's another side that deals with ob patients. It was very hard for me to see kids (12-15 yrs old) and pregnant in the same office. Most of the time the RE patients aren't on that side, but it does happen occasionally....Dr Nakajima was always patient and kind. He was the head physician over RE students so I had to answer most things multiple times which was annoying at first but was a good learning tool for all of us.

I appreciated Dr N's frankness and honesty. I always felt like I could ask any question even when I knew it must sound silly to him. He was our teammate thru the entire process for both of our children. I was initially concerned that there could be a communication issue, but he's a born and raised American and my worry was unrealized. I'm a huge fan and am grateful for all of his help.

Dr Nakajima was always patient and kind. He was the head physician over RE students so I had to answer most things multiple times which was annoying at first but was a good learning tool for all of us.

I have PCOS and top line insurance at the time so we skipped multiple IUI procedures since IVF was covered by my insurance. I don't remember specifics now. We were unsuccessful with 6 cycles of Femara and the single IUI round. However, the first IVF round we transferred 2 grade AB embryos. One stuck and I had our oldest son. We froze 3 embryos. Upon thawing, 1 embryo did not make it. The other 2 were transferred. Both stuck, and I miscarried 1 at 10 weeks. The 2nd was fine and I had our younger son. We were given the option to present an argument to transfer 3 embryos the first time, but we're comfortable with 2.

Angie was amazing. In fact, we still keep in touch today. She cried with me at negatives and cried with me at the positive tests.

The clinic moved facilities when I was a patient. In fact, I was the first IVF at the new facility. It's nice and provided much more space. As this is a university clinic, there's another side that deals with ob patients. It was very hard for me to see kids (12-15 yrs old) and pregnant in the same office. Most of the time the RE patients aren't on that side, but it does happen occasionally.

They offered IVF financing but I was very lucky and it was covered by my insurance. My fresh cycle procedures (egg retrieval and transfer) was $15,800. Frozen embryo storage fees were $250 a year. My frozen cycle procedure fees were $11,900. I think the IUI was $125.

I felt that Dr. Pagidas was a good doctor but I felt that due to the resident and fellow involvement, there was a lot of mixed information being provided. Her bedside manner was great and she was very friendly... I first did a medicated cycle with clomid and when it came time to do the trigger shot they did insemination the same day. It is my understanding that a minimum of 24 hrs should pass. I then did two unmedicated IUI cycles and the same issue occurred. Lack of involvement and assistance other than wanting to get us in and out.
The clinic [Univ of Louisville Women's Healthcare] was able to get us in quickly and scheduling was for the most part pretty easy. Clinic was clean and equipment was adequate. The clinic has multiple locations which can be a blessing or a curse depending on your preference... Nursing was very nice, however I again felt like my questions and concerns were disregarded or not important. Communication was also lacking with respect to care from nursing

Dr Bohler and his office couldn’t ever give us a straight story about how the process worked. We had problems getting our lining to grow and he just kept trying the same medications never once considering that maybe we needed a different approach from the norm....Completed surgery to remove a uterine polyp to get a “clean slate” for implantation. After retrieval, oral estrogen and then Letrozol were given in monotony varying doses for 8 months with no change in success with growth of the uterine lining.
Poor communication all around. We discussed our intent to do PGD for Huntington’s at our first visit. 2 months later we paid for the cycle of IVF. 2 months after that they [University of Louisville] came back and said we owed another $1500 for the biopsy for the PGD that they forgot to charge us for.... Seemed to never know what was going on and repeatedly gave us incorrect information resulting in false hope and poor planning.

I felt like a number with him [Dr. Bohler]. He was mostly just checking what his Fellows/Residents said was correct. Leaving all of the work to them. He was polite but didn't seem to show compassion for the very difficult time we were having...I never felt like there was a cohesive plan for my care. I managed all of the follow-up and spent more time with his residents/Fellows than him. While that wasn't an issue I didn't feel like anyone was an advocate for me. I was doing it all alone.
Nice that there were 2 locations [U of Louisville]. New location was much nicer than downtown location. At downtown you often has to check-in in the regular OB office which was full of pregnant people and their children...Conflicting information from nurses about follicle ultrasounds. Also not enough info on why that is important....[Nursing staff] Friendly enough but there wasn't 1 person on charge so I never felt like I was getting consistent info...$500 for IUI

Dr. Bohler seemed to dismiss my infertility issues and stated several times I was still young (32 at that time) so shouldn't worry....He didn't engage on a personal level and made me feel like my concerns were minimal. He was quick during all appointments and always seemed rushed... I was told I would have a full review with pictures/explanation at my post op appointment. I received neither and did not even see him only a RN and Doctoral student...
It took months to schedule an appointment. At each appointment I would wait close to an hour to be seen. Office [U of Louisville] is not very personal or welcoming with closed glass windows for staff....I was not billed for my surgery that occurred in 2015 until 2017. When I called the office to gain more information on the bill I was transferred from one area to another with never gaining a full understanding of the problem
